## Narrative

The spike for the Copper Sulphate Mix tank had been removed for cleaning of the basket underneath When positioning the spike with a forklift for reinstallation it slipped off the forks striking a bar that an Operator was holding one end of, causing the bar to strike the Operators foot. This caused serious contusions to the top of the right foot.
