Narrative

Taking apart feeder chain on 9 South Mains Section, beating the pin out with a sledgehammer. A small piece of metal came loose and struck employee on right side of nose, one inch below eye. Received 3 sutures. No issue with training, PPE, company rules/regulations, mining equipment or mining plans.
