Narrative

Employee unrolled a roll of 6"" ads hose. Upon returning to truck, slipped on an icy patch and fell on left shoulder/ribs. Employees left shoulder and ribs were very painful, therefore he was taken to the emergency room for x-ray. X-ray was negative for breaks.
