Narrative

Bolting off the right side of the miner in 19D when the bolt bent forcing his hand against the mast of the Joy miner bolting station. Received contusion to index and middle finger and fingers were splinted together temporarily. No issues with PPE, training, compliance or mining machinery. Poor hand placement was a issue.
