## Narrative

He is an experienced electrician conducting an elevator inspection.  He was wearing all of his protective equipment and his training was current.  As he descended the ladder from the inspection platform, the door to the hatch got away from him and closed onto his right hand causing a fracture of a metacarpal bone.
