Narrative

Details surrounding the incident are uncertain. Contract driver claims to have fallen approx 15' off metal stairs at loading area. Driver left the premises and delivered at destination approx 250 miles from plant. After unloading, driver reported to the trucking company next day. Texas Lime was made aware that this incident led to MSHA reportable criteria on 4/1/14. DOB Unknown
