## Narrative

EE was on board #4 Mac-12 and as the mantrip proceeded down the slope a few hundred feet the Mac-12 lost brakes. The mantrip traveled approximately 950 feet before being driven into the mine wall where it struck steel set supports. EE suffered two hairline fractures to pelvis.
