Narrative

Employee was making a mechanical splice on belt, while swinging a 3lb hammer to drive required rivets he strain/sprained his shoulder. PPE was not a factor/ mining equipment was not a factor/ mining systems was not a factor, compliance rules/regs was not a factor, miner proficiency may have been a factor.
