## Narrative

The IE was loading roof bolts on the offside of the bolter. When the operator started to advance to the next row, he heard the IE scream from his foot being ran over.  The IE was taken to the hospital and treated for a contusion on the top of his foot. The IE continued to work, and then on 4/8/14, IE was seen by Dr. and failed to report to work on his scheduled afternoon shift.
