## Narrative

On February 17, 2014 an employee came into the office and stated that he is having pain in his right elbow since early January 2014, while performing his roof bolting duties. He has been diagnosed with medial epicondylitis (Golfers Elbow). He received an injection in his elbow to relieve the pain on 4-8-14.
