Narrative

Employee was exiting a jeep when he stepped into a uneven place on the mine floor. He strained his lower back. He started missing work on 4/15/2014. No equipment involved. It didn't involve job skills, proficiency, training, or attitude. No PPE or protective devices on equipment were involved.
