## Narrative

Employee was connecting his jeep to a trash dumpster at the shaft bottom. He leaned over to place a chain through a clevis. As he stood up he felt a pain shoot through his lower back and down his legs. He continued to work until he started missing April 22, 2014 pending surgery for a bulging disk in his lower back.
