Narrative

Employee was setting a shield when it pick up the adjacent shield and the pontoon came down on his right foot pinching it. It resulted in a fracture. It didn't involve any compliance issues. It didn't involve job skills, proficiency, training, or attitude. No PPE or protective devices on equipment were involved.
