## Narrative

Employee was roof bolting when a piece of top coal fell and struck him on top of his head and right shoulder. He finished his shift and did not report the incident until 04/14/2014. His first shift missed due to the injury was 04/28/2014. An MRI revealed prominent disc extrusions in his upper neck.
