## Narrative

He was helping to fill the bulk dust tanks with the chute hose.  He was wearing all of his protective equipment.  After he dumped the last load of dust, he bled off the pressure and tried to remove the coupler from the chute.  He pulled too hard and struck himself on the mouth causing a laceration.
