## Narrative

The employee in question was working to weld a plate onto an overhead location. The employee had pre-positioned the plate against a ladder that was beside the ladder he was to ascend, the employee then reached down to grab the plate, and the plate began to slide, and was unable to catch the plate to stabilize it, causing the plate to strike the employee's left foot.
