Employee was bolting in the face of 1 left, while installing inside bolt of first row the immediate mine roof fell with debris coming back toward the operator striking employee in the left lower leg. PPE was not a factor, mining equipment was not a factor, mining systems was not a factor/compliance rules/regs was not a factor, miner proficiency may have been a factor.
