## Narrative

Employee was being transported in the #11 seam on a diesel ride in order to get his golf cart ride. While in transit, a small piece of gob fell out between roof bolts and hit the top of his head. This pushed his hard hat down on the right side cutting his ear and the side of his head. He was later taken to the local hospital where he had six (6) stitches to close the wound.
