Narrative

He is an experienced employee with 4 years experience. He was wearing all of his personal protective equipment. He was attempting to re-insert a relay bar into the pan line. He hooked a come-along to the relay bar and shield canopy. When he raised the canopy, the chain broke and struck his right thumb.
