Narrative

On 6/4/14 at 3:00pm employee was removing the top shell from the bottom shell of a cone crusher. While backing out push bolt his left hand was under the top flange. When he hit the impact gun trigger his left hand was pinched between the impact handle and flange. He was diagnosed with a hand contusion and placed on restriction as of 6/5/2014.
