## Narrative

Employee stated that he was operating the continuous miner in the face of the #5 entry.  He had taken the 1st sump in the cut sequence and had positioned the off standard continuous miner for the second lift when a section of draw rock fell from the top between the roof bolts and the rib, striking him in the back of the neck and left shoulder blade area.
