## Narrative

The primary hoist (#3) went down at 6:15 due to a mechanical failure. All personnel were accounted for. MSHA was notified at 6:38. Mine personnel found that air had leaked out of an accumulator through a leaking pet cock. This caused the oil in the tank to drain and cavitate the pumps. The problem was corrected and the hoist was up at 8:55am
