Narrative

Employee was walking down the # 5 entry between the shuttle car and rib where the trailing cable was 16"" of the rib. Tripped on miner cable and twisted left knee. EE was taken to hospital and released to return to work, continued to until work until 6-25-14 at which time the DR told them they would need surgery to repair the ACL and was taken off work.
