Narrative

Employee was checking his sights when a piece of rock fell from the mine roof and struck him on the head, left shoulder, and left arm. The injury resulted in a contusion. It didn't involve any compliance issues. No equipment involved. It didn't involve job skills, proficiency, training, or attitude. No PPE or protective devices on equipment were involved.
