Narrative

The employee was walking to the stacker belt when he stepped in a hole and hurt his left knee. X-rays taken at Davis Memorial Hospital were negative and the employee returned to work on his next scheduled shift. The employee continued having pain and was seen by an orthopedist, which recommended surgery on Monday July 7, 2014. *EE was operated on 7/7/14 and will be off work.
