## Narrative

A roll of conveyor belt accidentally became dislodged and rolled against the back of the employee's left foot resulting in multiple fractures of the foot.  This incident did not involve any mining equipment, nor any aspect of compliance with rules or regulations.  The employee failed to review his surroundings prior to moving the roll of belt.
