Narrative

While changing dozer ripper tooth, ee hit pin with 4 lb hammer and piece of metal embedded in left arm near elbow. Originally determined ok to not remove, but orthopedic surgeon recommended removal due to location of metal near joint. Surgery on 07/24/14. Ee full duty until that day.
