Narrative

Employee was installing a bottom belt roller and got his finger caught between the roller and the belt stand. The employee received a laceration on small finger. Root Cause: Concentration. Did not involve: rules, regs. compliance, clothing or protective devices, training or attitude. Proficiency may have been a factor.
