## Narrative

EE began to trouble shoot an electrical problem at the plant & after visual inspection determined troubleshooting in MCC was necessary. He then opened the transformer for visual inspection. Using a T5-600V multi-meter to test voltage, the meter exploded causing a flash incident. The transformer is 4160V AC. He received 2nd and 3rd degree burns to both hands and forearms.
