Narrative

Employee was attempting to tighten a 4"" pipe coupling in a vise. As he put pressure on the handles to tighten the vise the coupling slipped. This allowed the vise handle to free wheel causing employee's hands to slip and loose his grip. The sudden jerking motion injured his shoulder.
