EE advised us letter dated 8/4/14, was received by employer on 8/8/14, illness to lower back is related to wear & tear from 13 year tenure at Greystone Quarryes, Inc. Attached copy of letter for reference. E-mail, ee advised us, as per his doctors communication and the x-ray results, the correct date for the injury/illness is 6/17/14.
