Narrative

During part 50 audit, it was determined this injury as reportable. Employee was reaching for the winch cable when co-worker reversed lever on scoop pinching his hand between the bucket and ram. Cause of this injury was poor communications. Compliance with rules a factor, PPE a factor, and miner proficiency. Mining/equipment systems not a factor.
