During part 50 audit, it was determined that this injury was reportable. Employee was struck by a piece of rock that fell between bolts at the stage loader area. Cause of this injury was not pulling down loose top. Compliance with rules/regs. and miner proficiency could be a factor, mining equip/systems, PPE was not a factor.
