Narrative

During a part 50 audit, it was determined that this injury was reportable due to embedded object. Employee was changing bits and a piece of the hammer (sliver) flew off embedding into his forehead above his eye. Cause of this injury was using pick hammer as a tool. Compliance with rules/regs, mining equip/systems not a factor, PPE (Faceshield), miner proficiency may be a factor.
