Narrative

Miner was loading an I-beam onto a mule deck when he felt a pain in his lower abdomen. The miner was diagnosed with a hernia and started to lose time on 8-22-14. the following were not a factor in the incident: Rules, Regs., equipment, skills, proficiency, clothing, attitude, or protective devises.
