## Narrative

EE was installing rib bolts. When placing wrench on head of bolt, a piece of draw slate fell behind ee, causing him to move foreword. This , in turn, made the wrench speed up and push ee's rt hand into rib between the bolt head. EE received 12 sutures between thumb and forefingers. EE's lost time started 9/11/14
