## Narrative

While roof bolting on the left side of a continuous miner, the employee had pre-made a roof bolt assembly and placed it along the right rib. When the continuous miner backed up, the rib protector caught the roof bolt, which then struck the employee with some force, along the right side of the employee's head just in front of the ear and temple.
