## Narrative

Employee alleges that he strained his shoulder/upper arm/abdomen and ribs while mounting a Getman A-64 Rig. The employees stated that he slipped on the front step and lost 3-point contact causing the injury. The incident happed on September 17th however the employee sought medical attention on Monday, Sept. 22 six days later.
