Narrative

The employee was moving a door closer from one arctic (cooler style) door to another door. He placed the closer on the floor of the Sewage Treatment Plant and then stood on the ends of the closer with the actuating lever braced against the floor. At this point the closer slipped out from under his feet and snapped to the ""closed"" position.
