Narrative

Employee was struck on the top of the head by a chain hoist. Hoist was hooked to a belt hanger bracket and around a feeder conveyor chain that was fouled. The belt hanger failed while ratcheting the hoist resulting in the hoist striking the employee. Employee was transported to CAMC General Hospital, where he received two staples in his head and was then released.
