## Narrative

Undetected material slid down the T-bar and slid down the mass boom of the T-bar striking the light frame causing the rock to break into small piece striking him on the left shoulder. When he moved away he struck his head on the canopy driving his hard hat down on his left ear causing a laceration resulting in stitches.
