## Narrative

Employee was preparing to setup to transfer cement. When preparing to loosen bolts on the cement line employee had a very sharp pain in left shoulder and bicep. Was seen by Dr on 6/19 & 6/26. Dr reported he had slight tear in left bicep/shoulder. Could continue to work without any restrictions. Employee reported he was still in pain. Returned to Dr and surgery is 11/12.
