Narrative

EE was drilling hole in No 6 face on 1 Unit to install roof bolt and a piece of rock 13""x7""x3""thick fell from mine roof and struck EE on back of left lower arm. EE RTW on 10/20/2014 then went back off on 10/21/2014 for same injury and RTW on 11/3/2014.
