Narrative

He was installing plastic mesh with continuous miner when the roof strap they were installing fell and struck the ee's hand. No compliance issues involved. It didn't involve job skills, proficiency, training, or attitude. No PPE or protective devices on equipment were involved. The accident occurred because they were installing roof mesh and not watching where the roof strap was.
