## Narrative

Individual was lifting a metal plate on the Alpine Mine when experiencing a pull / pop in the left shoulder. This injury did not involve any compliance issues, malfunction of equipment not involved, did not involve job skills, training or attitude. Individual continued to work until 9/11/2014 when surgery to the left shoulder was performed.
