Put up ""I Beam - 4600 Galena, felt pain in stomach. To clinic diagnosed-pulled abdomen muscle. Complained of pain-October 29, 2014, to clinic, diagnosed pulled abdomen muscle. Came back complaining of persistent abdominal pain, back to clinic - diagnosed-hernia. On restricted duty November 11 scheduled for surgery November 19th.
