## Narrative

EE was cutting down a tree.  They were using forklift to guide the fall of the tree.  When they were getting the forklift in position, the forks hit a dead limb, a 1"" x 6"" piece of the limb fell and struck the ee on the ground in the left eye.  Had to have surgery on eye to repair.  EE was wearing safety glasses.
