Narrative

Employee was cutting a dust hose with a utility knife the knife slipped lacerating his thumb.Result of accident was employees lack of awarness. PPE was not a factor,mining systems was not a factor,compliance rules/regs was not a factor,miner proficiency may have been a factor.
