Narrative

EE was stepping down off of the stool for the Imhoff collection container, the employee stepped on hose in the walkway, twisting his ankle, this occurred on 12/01/2014. EE refused treatment at that time. On 12/02/2014 EE stated that his ankle was irritated and sore. He was seen at the clinic and given restricted duty.
