## Narrative

Injured was removing the balance weight from a motor using a box wrench when the employee felt a ""pop"" in their shoulder.  Employee was evaluated by medical staff with no treatment at time.  Surgery was later deemed necessary by personal doctor, day of surgery 12/5/2014 - 1st day away from work.
